Feedback / Suggestions / Grievance Form

The objective of this form is to provide any feedback / suggestions or to report any grievances for any individual or any department.

Any feedback received via this system will be sent only to Jigar on his personal mail id, and will remain confidential with him. No other employee or department will have access to these details; there is no database for this system, so data cannot be accessed through database by anyone.

Feedback can be given anonymously as well using this system, so that you identity will remain undisclosed. Hence the "Name" and "email" field have been kept Optional.

The feedback applications will be reviewed once a month, so in case of any urgent / important feedback, please ✔ "URGENT ATTENTION REQUIRED" field. The review weeks will be last week of every month. In case you ticked the "URGENT ATTENTION REQUIRED" Box, you can expect a response in earlier.
